I've encountered this bug a few times since starting to play on the server, however, it seems to be difficult for me to reproduce it consistently. In my cases I've seen it affecting the headgear items only, it has never impacted any other items that wear on the torso or legs/hands.

Yep, That's right.
I encountered that alot too
TBH I’m a bit unhappy with the way I scripted clothes loading/wearing. I’ll rewrite parts of the code soon to hopefully prevent or reduce any issues.
This will be fixed in the next update.
