How to claim donation rewards?

I have donated 5,00 GBP to this server, however I do not know how to claim the rewards in-game. How would it be possible for me to link my account to the donation made to the server?

Contact Mike related to Donation issue, you can't pm him on forum, but you can contact him on discord.
