Why was this ban/infraction wrongfully issued?   It is correct, I admit my mistake
What was the situation which lead to this wrongful ban/infraction? What was happening?   I entered the server when I was still banned, I made a mistake. I'm sorry, could you give me one last chance?

* NOTE *dan already responded to this report saying to wait for my ban to expire, the problem is that the ban is permanent

 Appeal after a month, stop making new appeals.
